How do I complete a Family Goal Plan follow-up?

1. Pull up the Family Goal Plan Overview page. You can find this page a few different ways:
   1. if you are assigned for this referral, you can find the reminder on the Home screen
   2. Reminders card on the Household Overview screen
   3. Family Goal Plan History section on Household’s History page.
2. On the Family Goal Plan Overview page, go to the History section within a Goal.
3. Next to the follow-up, click Complete. This will take you to the Complete Follow-Up page
4. Enter the Date Completed, Follow-Up Disposition, and any applicable notes and click Save
