Client Enrollment collects information related to Health Insurance. You are required to answer if the client has health insurance using Yes, No, or Unknown.
- Users will be required to select if the client has Health Insurance (Yes, No or Unknown).
- If the answer is yes, users will see additional fields become available.
- Health Insurance Type is required. (Direct-Purchase, Employment Based, Medicaid, etc.).
- Health Insurance Provider, Member Id and Medicare Ids are optional fields.
- Member ID can be entered without selecting a Health Insurance Provider.
- If Medicare is selected as the Health Insurance Type, user will see an additional field to enter the Medicare ID.
Users are able to enter more than one health insurance by clicking + Add Additional Health Insurance.
